The MAIB report has very little to offer that wasn't within the range of likely conclusions. It comes as no surprise to read that the owner and his wife were not reckless and irresponsible (as some suggested) but were simply unlucky enough to be in the wrong place when all of the holes in the Swiss cheese lined up. The self-evident fact is that the kill cord safety system did not work for the Milligans. The fact that it would have worked if they had used it does not invalidate that conclusion. An alternative or additional system may have provided an additional defence that would have prevented the accident.

The MAIB way forward - ensuring that use of the kill cord becomes "second nature" is simply not going to happen in a large number of cases. No action will become 'second nature' unless the circumstances in which it is to be performed occur frequently enough for habit to develop.

Second nature may develop in the case of a pro user but unlikely in the case of a leisure user who is on the water, on average, once a month or less.

I would say that my statement was based on fact rather than opinion. Kill cords are cheap & reliable if used correctly, much like seat belts & lifejackets. No one has yet proposed a viable alternative to the killcord that doesn't depend on either fanciful electronics or concepts, or hideous amounts of money. Most, if not all of the reasonable alternatives involve some form of user interaction, which brings us back to the killcord. If you read the whole thread, you'll see that all the arguments are circular & end up back at the killcord. The weak link in the system is the organic lump between the seat & the wheel, factor that out & you might have a chance, but whilst ever that variable exists in the equation there will always be the potential for collateral damage. As I said, there's always a better class of idiot & that includes me, I'm on record saying that I have forgotten to use the KC on more than one occasion. Show me a system that's idiotfool proof, reliable, affordable & I'm first in the queue
