Looking at the pic I'd guess the mounts failed. Strangely enough not unrelated to the reason I'm replacing the creaky (literally) old things on mine during the rebuild.......

Only "off transom" moment I've had was a Mariner 15 off the back of an 11' dory. No transom plate, and it just worked it's way up the fibreglass & fell off. Thankfully happened whilst getting on the plane so it literally just dropped off. The good thing was the deadman was on the engine & clipped to the back of my lifejacket belt (the one advantage of a boat that small!) and it pulled out as it went ofer the side, saving it a good drink of salt water. It was wire steering, and the shock loading pulled the steering colum off the floor! Thankfully it had a tiller as well, so after hauling it back onboard, bailing the cowl innsides & disconnecting the remotes, we carried on with the tiller......

We fitted a cast ally plate with a lip to prevent it happening again. A few(!) years later & a Hp upgrade and so far so good.........

seen a 3cylinder Penta come off a boat that was borrowing it to see how it performed. No thru bolts just the thumb screws. Came off at full chat-sucked in water, pistons came up against an incompressible medium and bingo- the hydraulic lock caused con rods to break, crankshaft to bend and wallet to suffer a severe amount of damage. ouch!
