I suspect the shock tactic does not work. Everyone was shocked by Padstow but it seems to have not had any real effect on kill cord usage so far.

Depends how graphic you are with it. However, I suspect being graphic enough to shock might make kids upset.

Maybe print both front and back with the kill cord one side and propellors Kill on the other, with a prop in a triangle?

Maybe look for an idea that is provocative enough to make people think, without being graphic or gory.

I'm not sure whether focussing on the prop is helpful, or whether it should all be about the kill cord. They are obviously inextricably linked, but the message needs to be really simple like "Kill cords save lives"

Or how about looking at safe sex campaigns for some parallels? If it's not on, it's not on!
