Lets evaluate the picture........

...in a little more detail. Surgical smock, collar, nice looking hands (not a stinky fisherman). Could be an RSPCA man - might not - Dog does not look at all distressed or wet ! No blood ! So why ?

Surely that hook would pull straight out with a big dog dragging through the water? yep I'd say fake as well. (at least I damn well hope so)Before we congratulate ourselves too much I remember seeing a heated debate on other countries animal welfare issues and a Spaniard resolutely defending bullfighting, all was going well until he came up with-
"I see you have a Royal society for the prevention of cruelty to animals, but only a National society for the prevention of cruelty to children, why is that?"

Ouch!

Why would the RSPCA use a fake picture?

IMHO, If a dog were rescued then the first thing they would do is get it to a vet and clean it up. The last person to pose with the exploited dog would be the fisherman!

Anyway, I've sent an enquiry to the RSPCA asking them to clarify the circumstances of the picture.

Now the picture makes sense... he's at the vets after being rescued. So if someone's rescued him and had the decency to take him to the vets, why the feck aren't the authorities taking any interest ? ...and at the same time why aren't we criminalising some of the barbaric practices closer to home?

I suspect that the photo was taken at the RSPCA or a vet's and that the Dog was sedated and the wound cleened and prepped for removal.
