Try the Apples store.

My son works in the one here in Sydney and they offer a very competitive replacement phone price if you hand in your old one even if it has been to the bottom of the sea.

You don't get the charger and the box as they assume you don't need it.

It is the same in the US, in Malaysia and in China so I am thinking I is a worldwide policy.
